A Claim Form is to be used to obtain payment for reimbursement of eligible costs.

A Quarterly Progress Report Form must be completed and sent to the Ministry of Community Services every quarter (March 31st, June 30th, September 30th and December 31st) until the project is complete. A Quarterly Progress Report Form is required even if the project has not started.

A Final Report Form must be completed and submitted upon project completion.
